POSITION AVAILABLE

POSITION TITLE:  Case Manager EMPLOYMENT STATUS:  Full-Time
REPORTS TO:  Residential Services Manager FLSA STATUS:  Non Exempt
WORK SITE:  Residential Facility & Off-Site Locations HOURS:  4-midnight Sun. & 1-10pm Mon. - Thurs.
WAGE:  $9-$13.50 Commensurate with experience & skill level  

 

JOB RESPONSIBILITIES:

  • Provide case management and supportive services for families residing in an emergency shelter
  • Assist adults and families in achieving self-sufficiency
  • Respond to crisis telephone calls
  • Maintain the operations of an emergency shelter program

 

ESSENTIAL FUNCTIONS:

  • Provide adult and family case management and supportive services
  • Conduct program intakes and assessments
  • Identify the strengths and weaknesses of adults and families
  • Establish service plans for adults and families utilizing development matrixes
  • Travel and transport individuals and families to off-site locations
  • Provide referrals for adults and families
  • Build and maintain networks and support systems for adults and families
  • Maintain the confidentiality of clients
  • Design and implement educational programming for adults
  • Attend staff meetings and in-service training
  • Other duties as assigned


KNOWLEDGE, SKILLS, AND ABILITIES:

  • Knowledge of family violence, homelessness, sexual assault and child abuse
  • Knowledge of crisis intervention techniques
  • Skills in written documentation
  • Skills in interpersonal communication
  • Skills in conflict resolution
  • Ability to carry out instructions in written, oral or picture form
  • Ability to exercise independent judgment

 

Training will be provided in the Family Development Case Management system.  Written and oral communication skills are critical requirements.

WORK SITE:  Residential Facility HOURS:  On-call

JOB RESPONSIBILITIES:

  • Maintain the operations of an emergency shelter program
  • Respond to crisis telephone calls

  • Arrange emergency and essential transportation for program participants
  • Ensure the safety of program participants
